for your own profile, feel free to use whichever prefix you prefer. For ancestors, Linda is correct - those are not normally used.

Your "Current Last Name" should be the last name that you are currently using. For ancestors' profiles, it's the last surname they used in their life (I've heard CLN referred to as the "died as" surname).
sorry - forgot to cover "Other Last Name" ... I've been married three times, and took my husband's name each time. So my "Other Last Name(s)" are Lavallee and Downs. My Current Last Name is Shifflett. My Last Name at Birth is Noland (and I do use the prefix Mrs. for my profile).

For my ancestors - who either couldn't spell, didn't care, or didn't ride herd on clerks recording their name - "Other Last Name(s)" include Nolan, Nolin, Knowland, among others.

Prefix as you stated are not normally used in the profiles.  Prefix should be for Titles, ie military, professional, etc.  

Last Name at Birth should be Birth Name / maiden name

Current Last Name is Married Name or currently used last name

Other Last name can be previously used names, such as married names, as well as other Spellings found on documents, etc.
