Biography

Hazel was born in 1907 at St John's, Newfoundland. She is the daughter of Herbert Chafe and Ada Cormack. She was living with her parents at St John's, Newfoundland in 1921.^[1] Hazel immigrated to Poe, Logan, Kansas, United States on Sep 1,1929.^[2]

She became a US citizen on Nov 10, 1950 at Los Angeles, CA.^[3] Hazel passed away in 1966 at Los Angeles, California. ^[4] She died on 26 August 1966, in Inglewood, Los Angeles, California, United States, at the age of 59, and was buried in Inglewood Park Cemetery, Inglewood, Los Angeles, California, United States.^[5]
