why will my birth source not register

Hi: I am having problem with the  birth source for Henry Vernon Smith. Why will the birth source not register. I have another profiles with the same problem.

 Can someone look at Foote -946 and help me out.  .. Thanks Marie

I fixed the issue for you someone removed the <references /> line under ===Sources===
