How can I properly add a source to Fitch-1800?

I have located Lois Fitch Tracy's birth evidence in "Vital Records of Norwich, 1659-1849" at https://tile.loc.gov/storage-services/public/gdcmassbookdig/vitalrecordsofno01norwi/vitalrecordsofno01norwi.pdf on PDF page 379/582.  I need help in adding the source information.  She is a descendant of William Brewster of the Mayflower (4th GGranddaughter) so I would like this done without error.  Thanks for the help.

What you are doing is just fine.  I played with several different styles before choosing one that works best for me.
