I think it says Dorothea Slamova or Dorothea Glamova, but I'm not sure which. What would the masculine form of this name be? Slama? Glama? Glam? Slam? Thank you in advance.

It is better to link to the whole page so that more of the handwriting is seen. Fortunately, this isn't that hard. It's Slámová (masculine Sláma).

Sláma is a common czech surname, it can be translated as a "straw".
