5 girl children, now all deceased.

I have found these sometimes in the Government Gazette through NLA Trove. An example is [[Dunk-542|Victor Rodney (Dunk) Henson (1909-aft.1959)]]. This requires that the name change was done officially.

I believe you can actually call yourself anything you want, as long as it is not in an attempt to defraud or to commit a crime, so there may be no paper trail.

If done by deed poll, then there should be newspaper notices. Bit harder to do these days, though, as one wants documentation for everything.
