I'm not seeing your sources here. Where did you obtain the full name, birth date and place, and death date and place? Can you point to sources for that information (e.g. baptismal records, tombstone, court records) and craft formal citations for each separate detail?
The "sources" page you linked to provides several examples for source citations, but you can also use the Chicago Manual of Style or, better, Evidence Explained by Elizabeth Shown Mills.
