Mildred G Butterworth was born on 14 March 1901, at Kennedy and Thomas Streets, ward 23, Philadelphia, Pennsylvania, to William H Butterworth and Annie W Penn.^[1] (The Find-A-Grave memorial reports her middle name is 'Gladys', but provides no evidence for that name.^[2] Perhaps the actual birth return has it.)

Mildred Butterworth died young.

On 3 August 1901, she died, in Philadelphia, Pennsyvania, of cholera infantrum. She was buried on 6 August 1901, in Cedar Hill Cemetery, Philadelphia, Pennsylvania, section 1 lot 264.^{[3] [4] [5] [2]}
