What is the WikiTree protocol if a Profile Manager passes away or has apparently not contributed recently?

How can one, politely, determine the cause of apparent inaction in such cases?

Jim, that is what I would like to achieve - but without upsetting anyone.

i.e. is there a lack of activity because the member is dead or, perhaps they are just not as active, recently, as they have been in the past?
Apparent lack of activity does not always indicate the member has died, and it would be premature to make such an assumption without certain proof.
I have had more than one case where a PM has been apparently inactive for years, but was quick enough to attend to a TL request, indicating they were at least monitoring their emails for WT.

If a profile manager is inactive for some time, and you have followed the required contact steps with no response from them, it then is time to file an unresponsive profile manager form.  Not a death of a member form.
Not active does not equal deceased, recently I submitted an unresponsive profile manager request, the PM had not been active since 2015, and did not reply to any communications. Yes, the profiles I was interested in were orphaned and I edited them as needed.

Then I submitted trusted list requests for a few other profiles  the person manages, and they got approved by the unresponsive PM within 24 hours!
