Help needed translating 1855 Polish? birth record.

Question

Hello, can anyone help me out with transcribing and/or translating this birthrecord of Emilia Lusch (upper right Nr.5)? the document should be from early 1855 Congress Poland, so it should be eighter Polish or Russian written with latin alphabet. Identifying even small parts especialy the exact date and Godparents, or details about the parents, confirming names and occupations would be very appreciated.
