Where can I find, or how do I create, a Category Info Box to create a new cemetery page?

I have created a few new cemetery pages, but not for a couple of years. I can't remember how to create the Category Info Box needed to complete the project. Any suggestions?

Required parameters are name=, parent=, location=, and coordinate=

Parent for US cemeteries is the county name in the form of |parent=XXX County, State
