This seems to be a record of his death.

HIs mother's name is very close to the one on this birth record.

Edit:  Sorry I forgot to put birth record

1910 census has a George Stafford, age 6, one of 8 children of Edmund (51) and Matilda (49). Edmund was born in Iowa and Matilda was born in Michigan. Edmund and Matilda have been married for 23 years. This is in Des Moines.

1900 census:

Edmond F Stafford b Jan 1859 Iowa and wife Tillie (short for Matilda) b Oct 1863 Michigan are living with 4 children in Julien, Dubuque Co, Iowa.

Edmond's father was born in Ireland and mother born Virginia; Tillie's parents were born in Germany.
The years married on both these census records suggests a marriage year of 1887-88 probably in Iowa.
Found marriage record of George's parents. They were actually married in Wisconsin. Gives full names of both of THEIR parents.
