How to enter first, middle names for a Catholic sister.

Nee Crescenz Wurst when she took religious vows her name changed to Sr. Mary Chrysostoma Wurst, SSND.

What name goes where?

BTW the sisters were named after saints.  In this case 

St. John Chrysostom 347 AD – 14 September 407 AD

Given birth name in First name field (or first and middle).  Religious name in Preferred name field.  (Some people put the religious name as a nickname, but it is not such.  it is a chosen and preferred name.)  It should then show up something like Emily Tempest (Sister Freda) Rich

Thank you.  That's exactly what I have.  I'm getting a "Suggestion, warning 737: Unique name in preferred name."  I also put in the bio the date that her name changed to her religious name.
Such suggestions can be marked as false with the comment "was a nun" or "religious name" (or similar).

I would suggest that her given and maiden names be placed in the proper space, Last Name at Birth, etc. Then in her biography section could be written: Upon taking her vows to the church on (date), she took the name Sr. Mary Chrysostoma Wurst, SSND.

Something to that effect...
