Phyllis Smith, the daughter of Alfred Caleb Smith and Mary Lana Waugh, was born 24 February 1920 in Etiwanda, California.[1] On July 18, 1940 she married Douglas Murdoch Johnson in Glendale, California.[2] They were divorced in 1982.
