Samuel Edward Phillips was born on February 10, 1943 in Oakland, Marion, Arkansas, son of Claude Neace Phillips and Hazel Ruby Weaver.^[1]

In 1950, Sammy was living in household of Claud N Phillips (36) in North Fork, Marion, Arkansas, USA. ^[2]

Sammy (35) and Patty LaPlante (25) were married in 1978 in Oklahoma. They were later divorced.

Sammy died on September 10, 1993 in Granger, Yakima, Washington, aged 50. He was buried in Zillah Cemetery, Zillah.^{[3] [4] [1]}
