Forest Richard Baham was born on 20 April 1882 at St. Tammay Parish, Louisiana, USA.^[1] He appeared on the census of 14 June 1900 in the household of his parents, Ancil M. Baham and Mary Celina (Hibard) Baham, at St. Tammany Parish, Louisiana.^[2]

He married Bella Baham, daughter of Ancil Baham and Adline Lee, circa 1913.^[3]
