"van der" are tussenvoegsels, not middle names. Middle names as used on WikiTree are the American middle names (example: John 'Fitzgerald' Kennedy). As said by Sieger, we do not use middle names in the Netherlands, only given names (voornamen). For example: Johanna Frederika Klasina de Jong: all three given names (Johanna Frederika Klasina) go in the "First Name at Birth" or "Proper First Name" box. (see also: https://www.wikitree.com/wiki/Project:Nederlands_Portaal/Beginners ) The tussenvoegsel is part of the last name, at WikiTree we put the full last name in the last name box (Last Name at Birth (LNAB), Current Last Name, Other Last Name(s)). Example: "van der Heide" will be put in the LNAB box. Make sure to use the last name that is in the birth record as LNAB, if no birth record is available use the oldest available record (baptism, lidmaat, marriage etc.), make sure you view the actual birth record to get the correct name. If a baptism record (and no birth record is available) has a child named Hendrik and he is a son of Roelof Hendriks, then use Roelofs as LNAB. You have to add the s (or sometimes sz) to the name. It depends on the region and time if it will be Roelofs, Roelofsz or Roelofsen. You can check the other records (baptism records of his children or marriage record(s)) for what (patronymic) last name he used the most throughout his life.
At WikiTree for place names we use "the name that was used in that place, at the time of the event you're recording", see https://www.wikitree.com/wiki/Help:Language_Selection I myself have not yet bothered with the correct naming of older place names (Republiek der Zeven Verenigde Nederlanden, Bataafse Republiek etc.) but please use Nederland, not Netherlands. Also use United States (of America) not Verenigde Staten (if you happen to come across an emigrated cousin or something).
If possible please use primary sources: a link to scan on Family Search or you could use openarchieven.nl or allefriezen.nl or alledrenten.nl (or any similar site). Please don't just copy/paste the url as a source but state what the source is [use brackets]. Don't just type "https://www.familysearch.org/tree/person/details/GQSQ-9K4" but type "Family Search, Family Search tree, Hindrik Roelofs profile". Note that this url is not a primary source and it will still take me (and others) potentially a lot of time to find the actual sources. I will give you an example of how I do it: see https://www.wikitree.com/wiki/Baas-582 This way at the sources section you can click birth, marriage or death and have a scan of the primary source available. You can easily take the Open Archieven url and past the 'bronvermelding' that is available on that page.
